Project Request Form

This form allows a member to request the leadership team to approve resources for a project. The information needed for this form creates the business case and/or charter which are synonymous at BSF. Projects are for a fiscal year and they are generally approved in August. A mid-year review occurs in March which allows the Leadership Team to re-evaluate approved projects and potentially approve projects for the remainder of the fiscal year.

What is the problem? State the root problem if able or why this project is necessary?

Describe the project and how it will resolve the problem?

Provide at least three of the sub-components, milestones or sub-deliverables of this project?

What are the metrics you will use to measure the success of the project? How will you know the project is successful?

Draw a clear connection between the BSF mission, vision, strategy or Strategic Initiative to the need of this project.

Identify known risks. What events could threaten this project?

When do you need to kick-off this project and start detailed planning with the project team?

This is the estimated delivery date of the project.

How much money has been approved for this project (add expense and capital budgets to make a total budget)?

If your entire budget is for this project, put "NA". Otherwise, put the major line number. You can get it from your Division Lead or TR.

Will this project require the gathering or storage of data during and/or after the project”?

Who are the groups that will be affected by the development and implementation of this project? Provide at least three.

Large projects need an individual to maintain documents, create schedules and refine data. In smaller project, the Project Owner is also the Project Manager.

These individuals and Divisions are necessary to complete the project. What other capacities to you need outside of your division?

Provide options for Leadership. For example, how would a gold, silver or bronze levels of this project differ. This helps leadership fit the project into organizational needs and improves changes of approval.

This is the most important block of the request. Why should Leadership approve this project when there are so many other important projects? How will the benefit of this project exceed the required cost of time, money and resources?
