This is the form you need to submit to the Central NM STEM Research Challenge in order for your project paperwork to be reviewed for prior approval by the Regional Scientific Review/Institutional Review Committee. THIS FORM IS ONLY SUBMITTED FOR PROJECTS REQUIRING PRIOR APPROVAL FROM A REVIEW COMMITTEE PER ISEF RULES where there is no local/school SRC/IRB as well as for ALL Vertebrate Animal projects.
