This submittal form is to request a DWSD flow test, which is needed prior to approval of a water tap for a fire suppression system.

Include only street number and street name, do not include Detroit or zip.

Example: 1234 Woodward Ave

(Note: fire line must be at least one inch diameter smaller in size than DWSD water main diameter)

Provide drawing of site that shows where the fire line enters or will enter the building, include the address, the tap size and North arrow.

Flow tests are valid for one year.


Flow tests are done in the order they are received. Flow tests will not be performed when the weather will be at or below freezing. The process will cause dangerous icing conditions.


Results will be emailed as soon as available. Requester is responsible for distributing flow test results to the project team as needed. Drawings/sketches attached to this form are for the purpose of conducting the flow test only and do not constitute an explicit/implicit formal approval by DWSD of what is shown on these drawings.
