NCHE General Payment Form Submission

Individuals must submit a W9 and Electronic Funds Transfer (EFT) form in order for New Venture Fund (NVF), National Collaborative for Health Equity's (NCHE) fiscal sponsor, to process payments.


Collecting W9s ensures that we are compliant with all IRS procedures. NVF will not report reimbursement expenses to the IRS and thus, you will not need to report these on your taxes.


EFT forms are required as check payments are not being issued. Be sure to scan or affix a voided check with a current address to the form.


EFT, voided check, and W9 forms should have the same name and address listed on all documents. Click the following links to access a W9 and an EFT form.

If you have previously submitted a W9 and EFT, you will not need to submit them again, unless there are updates to the information.

Please indicate if you are submitting your EFT and W9 to NCHE for the time or if you have previously provided NCHE with your W9 or EFT and would like to update your previous submission.

If you are submitting payment forms for the first time you must submit both an EFT form with a voided check and a W9. The EFT form, voided check, and W9 should have the same name and address listed.
