I give permission to the Student Athlete Consortium (“the Non-Profit”) and its staff, partners, and representatives to use and share any photos, videos, or audio recordings of me and/or my child(ren). These may be used for things like social media, websites, flyers, newsletters, advertisements, and other materials that support or promote the Non-Profit’s mission.

I understand and agree that:

• These photos or recordings may be used at any time and in any location.
• The Student Athlete Consortium will own the rights to the photos and videos and may edit or change them.
• I will not receive any payment or compensation now or in the future for the use of these materials.
• I give up the right to make any legal claims against the Non-Profit or its representatives for how the media is used, including claims about privacy or reputation.

This agreement applies to me and anyone who has legal rights through me, including my heirs or legal representatives. I confirm that I am either:

(a) an adult signing for myself, or

(b) the parent or legal guardian of the child(ren) listed below and have the legal right to sign for them.
