Social Determinants of Health RFI

Introduction

Social Determinants of Health (SDOH) are the conditions in the environments where people live, work, worship, and play that are known to affect a range of health and quality-of-life outcomes and risks. Using a whole-person health approach, the Department of Behavioral Health and Intellectual disability Services (DBHIDS) and Community Behavioral Health (CBH) invest in resources that address basic needs to help people achieve and sustain their whole-person health goals. The key areas of investment include employment, housing, food security, technology, and transportation.


In the 2021 Roadmap to Whole Person Health Report, the Pennsylvania Department of Human Services stated that “addressing SDOH is essential in improving health care value and addressing health equity.” SDOH assessment tools improve healthcare professionals’ understanding of an individual's social needs which can help them make more meaningful referrals for appropriate services and help identify social factors that negatively affect an individual's well-being. When choosing an SDOH assessment tool, agencies often consider a tailored tool relevant to the member population as well as programmatic goals.


CBH is seeking information from providers on the tools being used to assess SDOH as well as challenges and successes provider agencies experience when collecting, analyzing and utilizing data collected through the use of SDOH tools.


Through this Request for Information (RFI), CBH will gain insight into provider experiences using SDOH assessment tools with a goal to move toward enhanced standardization of SDOH assessments.


Terms of RFI

This RFI is for information gathering purposes only and is not intended to result in a contract with any respondent. CBH is seeking community insight and information prior to considering the development of a Request for Proposals (RFP).


While the goal of the RFI is to inform potential service development, this RFI does not commit CBH to publish a Request for Qualifications (RFQ), RFP or award a contract. The issuance of an RFQ or RFP as a result of information gathering from these responses is solely at the discretion of CBH.


Responses to this RFI will in no way impact any future

proposal submitted to RFQs or RFPs regarding this service. Those wishing to respond to any potential future proposals are not required to have first responded to this RFI.

CBH is seeking insight and feedback from in-network service providers in good standing with the City of Philadelphia and CBH.


Following the Submission

CBH will review all submissions and determine next steps based on information submitted.
