Please note that as per San Bernardino County Ordinance No. 4359, Section 19, 16.0213A,(a)(3) (effective July 1, 2019), research and analysis services being requested by an outside entity that requires the support of a Statistical Analyst or Epidemiologist will incur a $77.00 per hour charge for compilation and analysis services. ---------- Please complete the form below to provide our team the necessary information to begin processing your request.
