Your answers to the following questions will help our team create the best approach to support the implementation of this model within your Program. The Child and Family Health model is a holistic approach to positively impact the development of children with intellectual and developmental disabilities from 0-7 years old. The model consists of three components:

• Child Intervention (Young Athletes)
• Family Education & Support (Family Health Forums)
• Community Health (Healthy Young Athletes pediatric screening)

The CFH model integrates these three components, inviting children and families to engage in all of them over an 8-week period.
