By submitting this form, you authorize Prison Fellowship® to mail an Angel Tree® Christmas application on your behalf to the incarcerated parent. In the event the parent signs and returns a completed application, you’ve agreed to allow the children in your care to participate in the 2025 Angel Tree Christmas program and receive a gift and a message on behalf of their incarcerated parent. This form DOES NOT guarantee that your child will receive a Christmas gift through Angel Tree.

The incarcerated parent must do their part to complete and return the application that is mailed to them. We will not be able to confirm your child's participation in the program. An Angel Tree partner will reach out to you directly (usually October-December) if an application for your family has been received by Prison Fellowship.
