Overdose Fatality Review (OFR) Mentor Site Application
OFR mentor sites will serve as hosts for communities interested in planning and implementing an OFR. BJA anticipates that each mentor site will be asked to host approximately six to ten peer communities (typically teams of two persons). Visits will include opportunities for observation and peer-to-peer exchanges between the host and visitor communities. The 2024 mentor program will be on going from January 1, 2024 through December 31st, 2024.
The selected mentor sites should be prepared to work with BJA’s training and technical assistance provider (IIR) to host initial meet and greet call, provide opportunities to observe an OFR in action, host debrief calls with mentee, attend mentee OFR teams with IIR and provide feedback, attend quarterly check in IIR meetings, provide one-to-one feedback to IIR on a regular basis and provide ongoing support as needed.

If there is no formal committee or structure in place, identify the person to whom the OFR team reports recommendations and describe how those recommendations are tracked throughout implementation.
