Videoconference/Zoom Exception Request
Use this form to request an exception to EPPM 14. WSU VIDEOCONFERENCE (VC) COURSE POLICY. This workflow will seek Chair or Director approval and will be visible to the Office of the Provost.
When to use this form:
An exception is NOT needed when:

"Video conference courses will originate from and be delivered to VC classrooms located on the WSU Campuses or WSU Extension sites unless otherwise approved by the department chair. This ensure adequate technical and user support, creation of peer communities within the course, and provision of high-touch student support."
Permanent requests require additional documentation of an HRS Reasonable Accommodation or Provost approval.
Permanent exceptions require and HRS Reasonable Accommodation letter or Provost Approval letter.
List other classes impacted by this HRS Accommodation. Type "None" if there are no other classes that will be need this VC exception.

This must be the Chair or Director of this subject and cannot be a department coordinator or other person assisting the Chair.
The Department Scheduler will receive notice when the exception is approved.
VC(82) General University Classrooms (GUCs) are those spaces officially designated as videoconference spaces. These classroom spaces have been optimized for videoconferencing and have additional equipment to support an outstanding student experience. Other GUC spaces may be capable of limited videoconferencing, though do not provide an optimal student experience. The type of space impacts the Instruction Mode entered in Maintain Schedule of Classes. VC(82) spaces may use the receiving code "VR". All other spaces must use the receiving code "VS". More information about videoconferencing and instruction modes.
Academic Outreach and Innovation (AOI) provides technical support for videoconference classes taught in VC(82) rooms and other GUC spaces. AOI does not provide technical support for non-GUC rooms. The department agrees to provide technical support when videoconferencing is used in non-GUC spaces. AOI does not provide Zoom links for classes that are not taught on multiple
