Notice of Offence of an Academic Integrity Violation Appeal Form


This form is for use by students who wish to appeal a Notice of Offence of an Academic Integrity Violation.


You can only use this form if:


(1) you are filing an appeal for a Notice of Offence issued by a Faculty Dean, and


(2) the Notice of Offence was issued within the past Five (5) business days, and


(3) you can clearly demonstrate that the details of the Notice of Offence are inaccurate..


The appeal process is designed for students who believe they have been wrongly charged with an academic integrity violation, which they did not commit.


It is your responsibility to provide all necessary details and evidence to demonstrate that the Notice of Offence is incorrect. The Academic Appeals and Standards Committee will make its decision based solely on the information you submit in this form and will not seek additional details to support your case.


Your submission must contain a clear and objective appeal letter explaining why the Notice of Offence is incorrect.


Please note: The form must be submitted within five business days from the date the Notice of Offence was issued. Appeals submitted after the deadline will not be processed.

Please identify any members of the Academic Appeals and Standards Committee (AASC) who you think may have a conflict of interest adjudicating this appeal.


Under Section 9.5 of the Disciplinary Regulations on Academic Integrity, you must disclose any potential conflict of interest with an AASC member reviewing your appeal.


  • A conflict of interest exists if you have a relationship with a committee member that could affect an impartial review. Examples include:
  • A familial or marital relationship
  • A financial relationship (e.g., employer-employee, business partnership)
  • The AASC member is your course instructor for JURI 2106
  • A close personal friendship with an AASC member.

1. Appeal Letter


  • Your letter must objectively demonstrate to the Appeals Committee that you did not commit the academic integrity violation(s) outlined in the Notice of Offence.
  • Include all relevant details in your letter - a decision will be made based on the details you provide.
  • Appeals based on compassionate grounds cannot be considered, as stated in the Academic Integrity Policy. For further details, consult: Section 9 of the Disciplinary Regulations on Academic Integrity


2. Supporting Documents


  • Provide all relevant documentation to support your claim that you did not commit the violation(s) stated in the Notice of Offence - a decision will be made based on the evidence you provide.
  • Note: Do not attach .zip files.
