NEC's Responsibility Claimed or Credit Due Form

If all or part of your Complaint relates to responsibility claimed or credit due for projects or other work, the procedure described in this form must be followed and documented before your Complaint will be heard. Should the Complainant still choose to submit a Complaint regarding past violations of attribution, complete this form, and upload all its marked Exhibits.
