Departmental Equity Review Training Request Form

This form gathers information that will initiate the process for obtaining access to the training materials and Departmental Data Dashboards as part of the equity review process. DUPs, Chairs, Assistants and Admins supporting Chairs of the departments or other individuals who are working with their Associate Dean for Diversity are generally approved for access. Contact your Associate Dean for Diversity first if you haven't already.
