Paper Log Submission

Changes to the submission process have changed as of November 2024. Please be aware that all submissions are limited to one service type, 100 pages maximum per submission, and will be sent back to the submitter if there are corrections needed to process the log. Corrections will be sent back with the attached file to the recipient listed under "Submitter Email".


To ensure logs are accepted here are some tips:

  • Ensure there is only one service type.
  • Ensure the log is under 100 pages.
  • Logs that have only one attachment will be reviewed first, as combined files need to be split before we can process them. You may combine multiple students into one file!
  • Ensure your logs have the following required information:
  • Student's full name and DOB or ID
  • Date of Service
  • Start and Stop times for all sessions (except Transportation)
  • District name is on the log
  • Provider name is clearly written and a signature


All changes have been implemented to allow for more accurate reporting, efficiency in processing, and audit protection.
