Thank you for nominating a nurse for the DAISY Award! The DAISY award is a program to celebrate and recognize nurses by collecting nominations from patients, families, and co-workers. DaVita has partnered with the DAISY Foundation as a way to thank nurses for the care and kindness they provide.

We require the facility number and division name to properly identify the nurse you are nominating. If the information is missing or incorrect your nomination will not be processed.
