The City of Southlake is committed to providing equal access for all residents and visitors. The Americans with Disabilities Act (ADA) complaint and accommodation request form can be submitted for city programs, facilities, and activities.

Instructions: Please complete and submit form within 180 days of any incident and at least 48 hours prior to an event for an accommodation.

Within 15 calendar days after receipt of the complaint, the ADA Coordinator or his/her designee will respond to the complainant to discuss the complaint and the possible resolutions.
