A. Overview and Background

Overview: Establishing and sustaining coverage for the National Diabetes Prevention Program (National DPP) lifestyle change program in Medicaid is a key factor in promoting health equity for vulnerable populations at risk of developing type 2 diabetes. The Medicaid Beneficiary Enrollment Project provides an opportunity for states to receive funding and group-based technical assistance from the Centers for Disease Control and Prevention’s (CDC) Division of Diabetes Translation and the National Association of Chronic Disease Directors (NACDD) to increase access to and enrollment in the National DPP lifestyle change program for Medicaid beneficiaries. Background: NACDD, through support from CDC, has been providing technical assistance and financial support to State Health Departments, State Medicaid agencies, and their partners on the journey toward Medicaid coverage and operationalization of the National DPP lifestyle change program since launching the Medicaid Demonstration Project in 2016. B. Eligibility

The project is open to all states with two exceptions*: 1. *Existing Medicaid coverage is required for two of the four application tracks. See below, Section D, for more details. Medicaid coverage is required for tracks 1 and 2. 2. The State Health Department, Public Health, and/or State Medicaid Agency must serve as the fiscal agent. 3. Applications should have a strong focus on addressing social determinants of health and improving health equity. Applicants are encouraged to describe strategies for overcoming health related social needs to improve recruitment and retention. 4. Applicants are strongly encouraged to have commitments from both public health and Medicaid to accomplish the proposed scope of work. *Maryland and Oregon are excluded from this funding opportunity due to the provision of NACDD funding for their efforts to operationalize and sustain coverage, as part of the continuation from their participation in 2016-2019 Medicaid Coverage Demonstration Project.

 

C. Timeline and Funding Amount

The project timeline is August 1, 2021 to July 31, 2022. The funding ceiling is $100,000 per application. States will be notified of awards in August 2021 and obligation of the funds will begin August 1, 2021. States are asked to budget from August 1, 2021 to July 31, 2022. The budget should include subcontractors, as applicable. Payment of funds will occur through a series of quarterly scheduled invoices as determined by NACDD and the state. Provision of technical assistance will be available at start of the project period.

 

D. Application Tracks

Applicants must select one of the four tracks below. States are required to have existing Medicaid coverage for tracks 1 and 2. States are not required to have existing Medicaid coverage for tracks 3 and 4. Application Track #1: CDC-recognized organization(s):

Objective: The objective of this funding and technical assistance track is to prepare and support CDC-recognized organizations to recruit, enroll, and retain Medicaid beneficiaries. Project activities may include supporting CDC-recognized organization(s) to enroll with Medicaid and deliver the National DPP, to contract with a Medicaid managed care organization(s), to conduct outreach and increase awareness of the National DPP to Medicaid beneficiaries and health care providers, to develop strategies on recruiting /enrolling /retaining Medicaid beneficiaries, training health care providers about the National DPP and screening/ testing/ referral and to complete reporting and evaluation. Requirements: Application to this track requires partnering with a CDC-recognized organization(s). (a) This track requires States to have existing Medicaid coverage of the National DPP. (b) This track requires a Letter of Support(s) from the partnering CDC-recognized organization(s). (c) The CDC-recognized organization(s) may serve as a subcontractor. If the CDC-recognized organization(s) will be receiving funds as part of the award, those expenses should be enumerated in the budget.

 

Application Track #2: Umbrella hub arrangement (UHA)

Objective: The objective of this track is to build state capacity to develop or expand an existing Umbrella Hub Agreement (UHA) to increase Medicaid beneficiary access to and enrollment in the National DPP lifestyle change program. Project activities may include provision of technical assistance to an organization interested in becoming a Medicaid umbrella hub organization (UHO) on the process and how to enroll into Medicaid; to work with the State Medicaid Agency to accept an umbrella hub organization to deliver the National DPP; to implement a capacity assessment(s) to identify interested CDC-recognized organization affiliate(s)/subsidiary(ies); to develop a billing, claims, and reporting process or to contract with a billing, claims and reporting vendor; to contract with Medicaid managed care; to develop education materials for health care providers and Medicaid beneficiaries; and to complete reporting and evaluation. Note: While this project funding is limited to Medicaid, UHOs are encouraged to engage all types of payers. (a) This track requires the State to have existing Medicaid coverage of the National DPP. (b) This track requires a Letter of Support from the partnering UHO. (c) The UHO may serve as a subcontractor. If the UHO will be receiving funds as part of the award, those expenses should be enumerated in the budget.

 

Application Track #3: Medicaid managed care pilot and/or value-based offering:

Objective: The objective of this funding and technical assistance track is to build state capacity and improve managed care engagement to operationalize a Medicaid managed care pilot(s) and/or value-based offering. Project activities may include identifying eligible Medicaid beneficiaries; conducting member outreach; implementing a capacity assessment(s) with CDC-recognized organization(s); contracting with a Medicaid managed care organization(s); recruiting/enrolling/retaining Medicaid beneficiaries; and completing reporting and evaluation. (c) The Medicaid managed care organization(s) may serve as a subcontractor. If the Medicaid managed care organization(s) will be receiving funds as part of the award, those expenses should be enumerated in the budget.

 

Application Track #4: Other Strategic Methods

Objective: The objective of this track is for states to propose strategic methods to increase access to and enrollment in the National DPP lifestyle change program for Medicaid beneficiaries. Requirements: (a) This track does not require existing Medicaid coverage. (b) Letters of Support are required from all partners the proposal aims to engage. (c) Each partner’s roles should be clearly outlined. If partners are serving as subcontractors, those expenses should be enumerated in the budget.

 

E. Technical Assistance and Funding Parameters

This opportunity allows states to access funding and technical assistance from the Centers for Disease Control and Prevention's Division of Diabetes Translation, NACDD, Leavitt Partners, the Kem C. Gardner Policy Institute, and other subject matter experts, as applicable, to implement activities in their proposed work plan and achieve overall project goals. The specific nature and extent of this technical assistance will be mutually agreed upon by NACDD and the state and could include both direct engagement with the State Health Department, State Medicaid agency, and its partners as well as the development of resources. Awardees will have access to https://www.welldhealth.com/, a billing and claims platform. If Welld is leveraged in the proposed workplan, describe how it will be used to achieve project goals. Do not include Welld in your budget.

 

F: Awardee Expectations

1. The State Health Department and/or State Medicaid Agency, and partner organizations will participate in and share learnings through bi-monthly Communities of Practice with peer states. Awardees will be grouped into Communities of Practice based on their application track; 2. The State Health Department and/or State Medicaid Agency will participate in a two-part Medicaid Coverage Learning Collaborative to educate, share lessons learned, and provide feedback regarding the development, implementation, and evaluation of Medicaid coverage models. The Medicaid Coverage Learning Collaborative is a nationwide group of State Health Departments/Public Health and State Medicaid Agencies that have been actively working towards and/or have achieved Medicaid coverage of the National DPP lifestyle change program; 3. The fiscal agent provides quarterly reports of Medicaid beneficiary enrollment; 4. The applicants will commit to enroll a minimum of 50 Medicaid beneficiaries into at least one session (not including session zero); 5. The fiscal agent's program manager provides quarterly progress reports on work plan deliverables. Items to Note: 1. While funding can be used to pay for information technology (IT) and data analytics, CDC, NACDD, Leavitt Partners, and the Kem C. Gardner Policy Institute will not be providing technical assistance on IT-related items. 2. While funding may be used for hiring staff, there is no guarantee that funds will be available the following year. 3. Funding may not be used for translation of CDC-approved curriculum.

 

 

Selection Process

The selection process is competitive, and awards will be made based on the quality of the proposed work plan, corresponding budget, public health and Medicaid joint commitment, and adherence to the technical assistance and funding parameters outlined in Section E. 1. The work plan must be completed and submitted in the Smartsheet application; 2. The project budget should reflect a project period of August 1, 2021 to July 31, 2022 and should not exceed $100,000; 3. The project budget can be submitted in either Word or Excel format and should be a high-level estimate of project expenditures. Subcontractor expenses should be included, as applicable. The project budget should be uploaded into the Smartsheet application as an attachment; 4. Letters of Support(s) are required according to the application track selected. The Letter of Support(s) should be uploaded into the Smartsheet application as an attachment(s); 5. Do not include travel in your budget. Any state travel that is applicable to this opportunity will be supported by NACDD; 6. Per NACDD policy, there is an indirect cap percentage rate of 20%; 7.
