OneGuard Solutions Order Form

OneGuard Solutions is a "Do It Yourself" teeth & bite acquisition kit used to fabricate various oral appliances. Each desired appliance comes with 1 OneGuard Solutions kit to aquire the impression of the teeth and the accurate relationship between the top and bottom teeth referred to as "the bite". To purchase the kit and your OneGuard Solutions appliance product, we need you to read and understand important information to make the right decision. If you agree to purchase our amazing OneGuard appliance, either a SportGo! mouthguard, TMDGo! device, NightGo! stop the grinding device, Smile Refyne smile makeover appliance, or the SleepGo! get some real sleep device, we will follow the next steps EASY 3 STEPS as described below.


  1. Once you receive the kit, you will have a choice to schedule a virtual appointment with our OneGuard Solutions specialist to guide you through the steps to use the teeth & bite acquisition kit or schedule an In-Person appointment to be guided to use the kit.
  2. You will package the contents and ship them back to us in the included shipping materials.
  3. The appliance will be fabricated by our specialty dental lab, and the appliance will be shipped back to you within 14 days.

After purchase of the OneGuard Solutions teeth and bite acquisition kit, you have the option to

  1. DIY-watch an instructional video on our website to "DIY" do it yourself
  2. or be seen for an In-Person appointment in our office for a OneGuard Solutions team guided session.
  3. or be seen for a VIRTUAL appointment with our OneGuard Solutions team guided session.
