Requests for extensions and appeals

General practices and accrediting agencies can use this form to submit a request for the following:


  • Additional time for assessment process;
  • An extension to their period of accreditation awarded, either in advance of the expiry date or retrospectively;
  • Appeal a decision by the Commission;
  • Appeal against an approved accrediting agency.


All applications will need to provide a strong rationale accompanied with evidence and documentation in compliance with the requirements outlined in Advisory GP23/02: Requests for extensions and appeals.


Following the submission of a request, the Commission will review all the evidence submitted, collect further evidence from the general practice’s accrediting agency if required, and may seek advice from other relevant parties such as the General Practice Accreditation Coordinating Committee, prior to determining the outcome of the request.


Requests can take 10 to 20 business days to be processed, dependent on the type of request submitted, evidence provided, and the need to seek further information.

The rationale must meet the definition of an extenuating circumstance in Advisory GP23/02: Requests for extensions and appeals.

The rationale must be accompanied by evidence that the relevant accrediting agency did not comply with Commission specified policies, processes and/or directions relating to the assessment conducted.

General practices and accrediting agencies must determine what kind of evidence will be submitted. It is not intended that evidence be exhaustive, but it should be conclusive.

The Commission will only consider applications submitted within five months of a general practice’s current accreditation expiry date.

This is the date the general practice last completed an on-site assessment under the NGPA Scheme.

This is the total number of General Practitioners (GPs) involved in the delivery of health care in the general practice, regardless of employment or contractual relationship. Each General Practitioner is counted only once. Format: Number (e.g., 4)

For the purposes of this form, FTE is defined here as "the number of hours worked by a GP in the practice. One FTE is equivalent to 38 hours per week"


FTE is calculated by the total number of hours worked by all GPs in the practice per week over GP headcount, for example: 3.5.

This is the total number of other clinical (e.g. practice nurse) and administrative staff (e.g. practice manager, receptionist) who support the delivery and administration of health care in the practice. Each person is counted only once. Format: Number (e.g., 1)

For the purposes of this form, FTE is defined here as "the number of hours worked by support staff in the practice. One FTE is equivalent to 38 hours per week".


FTE is calculated by the total number of hours worked by all support staff in the practice per week over support staff headcount, for example: 3.5.

Please include dates of communication between the general practice and the accrediting agency regarding any issues relevant to this request.

Applications that do not meet the criteria of the Advisory will not be considered.
