GAP Request Form

 To submit the Grade Adjustment Policy (GAP) Request Form, I have:

1. Read the policy in the AU Bulletin.

2. Completed the Student Grade Adjustment Acknowledgement video and test with 100% accuracy.

3. Spoken with my advisor about the appropriate course to GAP and the reasons why I would like to GAP a course.
