GEA Sick Leave Bank: Opt Out Request

The Certified Sick Leave Bank is a benefit administered by the Greeley Education Association (GEA). Employees are automatically enrolled at the time of hire unless they choose to opt-out using this form. Please refer to the Master Contract (Article VIII, letter M, Sick Leave Bank) for more information, or contact GEA for specific questions.

Sick Leave Bank Opt Out Request*

By signing this form, you acknowledge that participation in the Sick Leave Bank is voluntary. Unless you decline participation below, you choose to enroll in the program and authorize Greeley-Evans School District 6 to transfer one day of sick leave to the GEA Sick Leave Bank and transfer an additional day whenever the Sick Leave Bank drops below 300 days.
