Parking Permit Application

Complete this application only if you are NOT Faculty/Staff using payroll deductions for payment.

-------------------------------- FACULTY & STAFF -------------------------------- If you have chosen to pay for your parking permit in full, and not utilize the payroll deduction system: 1. Purchase your parking at the Bursar's Office and retain the receipt. 2. Complete this application, uploading the required electronic copy of your receipt (a clear picture, scanned image, etc.) 3. DPS will contact you and set up an appointment; have your valid Driver's License, TSU ID, Vehicle Insurance Card, and License plate number with you, when you visit the West Garage. *The process of paying for parking permits via payroll deductions has not changed. The process above only applies if you are choosing not to use a payroll deduction to pay for your parking permit. -------------------------------- STUDENTS: -------------------------------- Student Permits $40.00 per semester Parking permits for students residing in Tierwester Oaks or University Courtyard, respective lots cost $40. An additional permit, allowing additional parking access to all of the campus Student Lots (example S-1 etc.) costs an additional $40.00 = Total cost of $80. * Any student requiring a Handicap TSU permit must show Handicap Placard and Driver’s License. Vehicle registration is required for DV plates. * If you are not driving the currently registered vehicle for unforeseen reasons, you may obtain a temporary parking pass at no cost for up to 2 weeks. You must provide make, model, and license plate number of the temporary vehicle. Any unregistered vehicle parked on TSU campus will be subjected to ticket and tow at the owner’s expense. Campus Student Parking Areas: S1, S3, S4, East/West Parking Garage Mandatory Documents required to obtain a Parking Permit: 1. Paid receipt from Bursar’s Office or attach online payment receipt below. 2. Valid Driver’s License and Current TSU Student ID 3. Vehicle Insurance and License plate number 4. If renewing, must have existing TSU Permit with you at time of renewal

The QR Code permit is for my personal use only and are not to be transferred for any other individual’s use.

The QR Code emailed to me will not be used to allow any other vehicle to gain access anywhere else on campus. I understand that violation of this term will result in the de-activation of my QR Code and I will be subjected to a $25 re-activation fee payable at Student Accounting located on the 1st floor in the E.O. Bell Building.

and agree to maintain this card for the duration of my tenure at TSU. I understand that this card will de-activate at the end of the last period for which I have paid for a campus parking permit (decal). I further understand that upon purchase of the school year or summer session parking permit, this card will be reactivated for that period. I understand that I will not be issued a new access card for each permit period. Parking Permits are to be renewed every school year.
