Stage 1 and 2 Research Registration Form

This registration process applies to Stage 1 and 2 research that must be completed on-campus, in the field, or involving face-to-face human subjects research. Researchers conducting work remotely, such as theoretical and computational work, or non-face-to-face human subjects research (surveys, mailings, etc.) do not need to submit this form. Prior to completing this form, you are expected to download and review the Stage 1 and 2 Research Protocols: https://research.uoregon.edu/sites/research2.uoregon.edu/files/2021-05/Stage1%20and%202%20Protocols.pdf We ask you to upload the document to this form in order to streamline processes for sharing the protocol with your team members. All researchers must implement the protocols described in the first section of Stage 1 and 2 Research Protocols: Research Team, Activities, and Safety Protocols. There are 2 additional sections applicable for fieldwork (not involving human subjects) and face-to-face human subjects research. Please review and implement as applicable to your research.
