Owner/Agent Pre-Inspection Request Form
A pre-inspection allows AMHA to inspect a unit before an RTA is submitted to shorten the time it takes to lease up a tenant. The purpose of the pre-inspection is to prepare an Owner/Agent for future HCV participation in accordance with HUD inspection requirements.
The unit must be vacant for the inspection and remain vacant until leased up with a voucher holder for the pre-inspection to remain valid.
If the pre-inspection passes, it will be valid for 90 days. If an RTA is processed during this time, no additional inspection will be required.
If the pre-inspection fails, then an RTA must be submitted for another inspection to be scheduled.
By completing this form you are requesting that the Inspection Department schedule a property for pre-inspection that meets one of the following criteria:
1. The unit is in an eligible zip code: 44056, 44067, 44087, 44141, 44202, 44221, 44223, 44224, 44236, 44264, 44286, 44301, 44319, 44321, 44333 and 44685.
2. The unit has accessibility features for persons with motor/mobility, auditory, or visual impairments such as an accessible building entrance on an accessible route, doors usable by a person in a wheelchair, grab bars, switches/outlets/thermostats in accessible locations, doorbell lights, specialized smoke alarms/carbon monoxide detectors, specific contrasting colors on steps/countertops/sink/door knobs, etc.
