Site Management Request Form

Sponsors of the Summer Food Service Program should complete this form to add, move, rename, or inactive SFSP sites in the the Colorado Nutrition Portal. Requests must be submitted by the sponsoring organization or School Food Authority. CDE Nutrition will complete requests in 5-7 business days and notify the contact when complete.

For National School Lunch Program site management requests, complete the NSLP site form.

To request changes for a Child and Adult Care Food Program site, please contact the Colorado Department of Human Services at cacfp@state.co.us.
