Faculty Appointment Intake Form

Information collected through this form is used to compile the Faculty Appointment Form which is routed via DocuSign for approval. Faculty are not authorized to begin working until the appointment form has received all needed signatures and final approval by SJSU Research Foundation. This form should be completed for each faculty member who needs an appointment form for an MTI project. Before completing this form, make sure to review your project budget -- faculty members need to know their current hourly rate and a breakdown of planned research hours by semester. Appointment forms can be submitted for a maximum of 4 semesters. This form should only be used for SJSU faculty members. There are different intake forms for student assistants and non-faculty researchers.

To calculate a faculty member's hourly rate, take your monthly base rate (check via MySJSU if needed), multiple by 12, then divide by 1360.
