By submitting this form and taking delivery of equipment from the MESH Computer Lab, you agree to return equipment in the same condition as when checked-out, and to report malfunction or any other issues with the equipment, on the Sign-In form, Immediately upon return. The device # is found on a green sticker on each cart, and on the back of each display.
