This program is designed to assist adult Tribal Members with financial obligations such as: housing, utilities, food, clothing, education, transportation, and cultural expenses. Each eligible adult will receive payments of $2,000 per month, unless otherwise directed by Tribal Council, which may include additional payments. Note: Adults receiving assistance who are represented by a Representative Payee under the Tribe’s Representative Payment Program will need to comply with spending guidelines and receipt requirements under PTC 1.12 Representative Payment Program Ordinance.
All original forms must be returned inside the enclosed return envelope, online, or in person to the Tribal Per Capita Department. Upon receipt of a properly completed application, we anticipate that it will take ten (10) business days to process and begin mailing/depositing checks. No GWE check or payment will be released until this application is completed. If you are already enrolled in direct deposit, your check will continue to be deposited directly into your account on file.

I certify that I am an enrolled adult member eligible for assistance under the Puyallup Tribe of Indians General Welfare Assistance Program hereby confirm that I understand that benefits under the Safe Harbor Program are limited to those that fit within the specific purposes described in EXHIBIT A. I am applying my monthly and any other general welfare assistance payments solely for one or more of the following qualifying expenses (please check all boxes that apply):
I certify that the above information is true and correct, that I will use my assistance payments for the above purpose, that the expenditure was reasonable in amount, and that the benefits applied are consistent with Tribal goals and Program purposes to promote the general welfare. I understand that it is my responsibility to retain adequate records if necessary to prove the amount of my Safe Harbor expenses as described in EXHIBIT A. I understand that if I am not able to substantiate qualifying expenses at least equal to the general welfare benefits I received during the year, that the Tribe must report these benefits as taxable.
