Public Records Request Form
- This form may be used to request information from this office. Once you've completed this form, we will review it and process your request as soon as possible. An active email address is required to complete this form. Do not send electronic mail to this entity if you do not want your email address released in response to a public records request. Instead, contact the office by phone at (407) 836-2070 or in writing, P.O. Box 562001, Orlando, Florida 32856.
- Please be advised that if extensive resources and more than 30 minutes are required to fulfill your request, a service charge may apply in compliance with Chapter 119 of the Florida Statutes. The service charge is in addition to any charges for the actual cost of production or duplication. It includes time spent searching for records, inspecting the records to be produced, and redacting any confidential information before production. A detailed estimate will be provided for your approval before fulfilling any public records request requiring a service charge.
- If extensive time or material costs are needed, payment will be required in full before processing the request (F.S. 119.07). Payments may be mailed, dropped off in the ballot/mailbox in front of our main office, or made in person.
- Supervisory assistance may be required to fulfill a public records request for documents or inspection of records.
- Please note the request processing time may be extended during an election.
- Pursuant to Chapter 119 F.S., when making a public records request, you are not required to give your name, explain why you are making a request, or submit a request in writing. Requesters are welcome to use this form to ensure clarity of your request, to assist in quickly processing your request, and to provide a way to contact you.
