Customer Name*

Plate Number*

A Plate Number is needed to locate the file in the system. Do not use an Order Number.

Bank Logo ID Number*

Enter the Bank Logo ID Number, if unknown use the link provided above to access the Bank Logo database.

Name of Bank

A bank may have many variations of there logo The Bank Logo is pulled by the Bank Logo ID number and the Bank Name is only used to verify the correct logo is located.

Standard Logo ID Number*

Enter the Standard Logo ID Number as it appears in Standard/Cutbook Logos
