VIVINT MEDIA PARTICIPATION RELEASE By signing this Release, either for myself or as the legal guardian of a child, I agree to allow Vivint, Inc. and its subsidiaries to use any of the following Content: 1) my testimonial, including quotations I may provide or adopt and/or 2) my video or photographic images (collectively referred to as “Content”) for use in advertising and marketing materials online, in print, broadcast, social media, internal, video, or photography, and any other legitimate business purpose (each category a different “Medium”). I agree to allow Vivint to use my Content to the effectiveness of Vivint equipment, employees, and monitoring services. I understand that, If I elect not to allow my Content to be used by Vivint in any specific Medium, I should not upload personally produced Content or have indicated my desire to share any personally created Content with Vivint directly through this specified channel or any other means of communication such as email, phone, or other social media sites. By uploading or sharing content with Vivint, I hereby release Vivint, its subsidiaries, officers, and directors from any claim, action, suit, or demand arising from using my Content as outlined herein including, but not limited to those grounded upon invasion of privacy, rights or publicity or other civil rights, or for any reason in connection with Vivint’s authorized use of my physical likeness, persona, and sound in the picture as herein provided. Vivint will not release your contact information or any other personal information without your permission. If giving a testimonial, I acknowledge that at the time of giving my statements, I have received no compensation for giving such testimonial and have no agreement, express or implied to receive compensation in the future. Rather, I have given, or plan to give, such a testimonial because I believe the words in any statement given to be an accurate indication of my true thoughts and feelings about Vivint and the services I have received.
