System Request Form

We’re here to help! You can use this form to submit requests related to Better Impact or FiveCRM! For example, you can use this form to:

  • Schedule a meeting or training session
  • Request resources
  • Ask a question
  • Share an idea!


Better Impact specific items may include requesting help to:

  • Update the content of your public pages
  • Change volunteer applications
  • Set up your volunteer descriptions
  • Create custom reports


FiveCRM specific items may include:

  • Request changes to your FiveCRM system (e.g., adding a new text box to the recoveree account details panel, adding a new drop-down option to the recoveree contact tab, creating a new report). Please note, FiveCRM system change requests will be reviewed quarterly.
  • Create a FiveCRM username and password for staff, recovery coaches, or telephone recovery support volunteers. (Please note, it will take up to 5 business days to create new accounts)
  • Deactivate a FiveCRM user
  • Reset a FiveCRM password

In 1-3 sentences, please tell us about your request. For example,

  • "Please create a new TRS volunteer user profile for Brad Pitt in FiveCRM. He only needs access to TRS calls that are scheduled for the day."
  • "Angelina Jolie is no longer a volunteer with our RCC. Please deactivate her profile in FiveCRM"
  • "We want to start using the custom reporting features in Better Impact. I'd like to request a training session to learn how this feature works and talk about creating custom reports for our RCC"
