Disbanded Club Form

In accordance with Cheshire County FA Membership rule 3 c), you are required to forward to the County FA office, all books, papers and vouchers and an audited balance sheet showing the financial position of the Club at the date of disbandment.


For the avoidance of doubt, books, papers and vouchers shall include;

1.    Bank Statements for the last two years.

2.    Minute Book and Constitution.

3.    Members of the Club for the last two seasons (including players).

4.    Receipts and Disbursements.


Please complete this form and upload the above listed documents. The above requirements are to be submitted to the County FA by the date outlined in the accompanying email; failure to comply will result in a charge of misconduct in accordance with CCFA Membership rule 8.8.
