Youth Program Preliminary Registration Form
Youth Programs are University or Auxiliary sponsored or co-sponsored activities/events that include individuals under the age of 18-years-old (minors), who are not matriculated University students. Youth Programs also include any on-campus activities/events that are organized by a Third-Party.
Youth Programs do not include the following activities/events:
- Activities/events at which parents, guardians, or affiliated program leaders (e.g., schoolteacher, counselor) are responsible for the care, custody, and control of minors
- Activities/events that fulfill credit requirements for a degree program
- Private, personal events (e.g., birthday parties, weddings) that occur at University facilities
- Events open to the general public (e.g., intercollegiate athletic events, concerts, class tours)
- Academic programming where students interact with minors as part of their coursework
- Off-campus academic internships and service-learning opportunities
Youth Program Directors are required to register their programs on an annual basis and must be approved by Risk Management before program activities can commence. Failure to register programs in a timely manner may cause program delay. Use this form to register new or recurring programs.
