What is the program’s overall focus? What kinds of services will be provided? Ex. substance use, suicide prevention, support groups, family supports, etc.

 

Which EBPs will be used? EBPs may be among those listed in the two menus found in the RFP. Other EBPs and strategies may be selected if jusitification is provided.

 

For Priority EBPs only, indicate the number of staff members requesting EBP training. Traing costs associated with priority EBPs should not be in the applicant's budget, but the applicant may budget for associated staff salaries/stipends. If no staff members will be trained in Priority EBPs, enter 0.

If applicable, provide Behavioral Health license number. Please indicate if services will be delivered under a group practice license or an individual license. If a license is not required for these services, please enter N/A. For reference, see Title 7.5, Subtitle 4 of the Health-General Article of the Maryland Annotated Code and Title 10, Subtitle 63 of the Code of Maryland Regulations (COMAR).
