If awards are made in December 2023 and initial distributions of funding are made in January 2024, how soon could grant-funded services begin? Ex. February 2024, August 2024, etc.

What is the program’s overall focus? What kinds of services will be provided? Ex. substance use, suicide prevention, support groups, family supports, etc.

Which EBPs will be used? EBPs may be among those listed in the two menus found in the RFP. Other EBPs and strategies may be selected if jusitification is provided.

For Priority EBPs only, indicate the number of staff members requesting EBP training. Traing costs associated with priority EBPs should not be in the applicant's budget, but the applicant may budget for associated staff salaries/stipends. If no staff members will be trained in Priority EBPs, enter 0.

If a license is not required for these services, please enter N/A.
