Strategy and Program Management Office Intake Form

The UCOP SPMO designed this Intake Form so that perspective clients can provided our office with the information needed for the consideration of a proposed initiative, program or project.


The form is an opening step in a consultative process. We ask that you complete the fields with as much detail as possible, but understand if some information is not yet fully known. If you are not able to complete a question, please provide your best guess or state "unknown".


Once the intake form is submitted, the SPMO will be in contact within three business days. From there, the next step is a joint conversation to better understand and confirm the details provided before the SPMO reviews against defined criteria and any final determination is made.
