Note: Where a person has a medical or other exemptions or medical contraindications to receiving a COVID-19 vaccination you may submit that exemption for our consideration to info@skopeconstructions.com.au. However, until you are formally advised otherwise your staff member will not be able to access our aged care site(s).

Return to this form and upload. Ensure the following criteria is entered when carrying out your application: 1. Ensure “Unsupervised Access” is selected; 2. Check type select: “Paid Employment” 3. In Occupation to read enter: “Contractor working in Aged Care Facility” 4. In Proposed place of work enter: Aged Care Facilities”
