This form is used to make an appointment with an Academic Advisor to discuss your Academic Performance Improvement Plan and how it can be tailored to best suit you. They’ll work with you to understand why your studies have been affected and identify relevant support services.

This interview will be held online over Microsoft teams and usually runs for 30 minutes.
