Please use this form to file an official complaint about a safety or sanitation issue at a restaurant or other food service facility in Washtenaw County, Michigan.

Please do not use this form to submit foodborne illness or "food poisoning" complaints. If you believe you got sick from food you ate at a restaurant or grocery store in Washtenaw County, please call the Environmental Health Division at 734-222-3800.

We do not handle customer service complaints like rude staff, incorrect orders, or slow service time. Please report these incidents to the facility’s management.
