This form should be submitted on behalf of a student driver to authorize use of the State of Idaho Rental Contract.

The event or activity must be on behalf and in support of University business as defined in section 3.8. Travel Policy 6180: “Official University Business or Business Purpose. An activity that directly carries out Boise State’s mission of instruction, research and service or that provides support to the University’s instruction, research and service activities.”

The student must complete applicable Drivers Training offered by the University before submission of form and driving under the rental contract.

A separate form must be submitted for each trip and rental under the State of Idaho's Automobile Rental Contract.
