All personnel listed on a UAS Flight Applications must first register with this form, including:

  • Pilot
  • Observer
  • Ground Station
  • Ground Crew
  • Trainee
  • Instructor

Please complete separate submissions for each person.


Note: If you are registering as a pilot, please have a copy of your Remote PIlot in Command (Drone Pilot) Certificate ready to upload at the end of this form.


Approval of this form will provide you with a registration # for this operator. Retain this record for use on future flight applications.
