Chapter Health Care Facilities Innovation Conference Registration Fee Voucher Request Form

Please complete this form to request the Health Care Facilities Innovation Conference registration fee vouchers that were awarded as part of the ASHE chapter award program. ASHE encourages (but not requires) vouchers to be distributed to individuals who are:


  • An active ASHE Member
  • A first-time Health Care Facilities Innovation Conference attendee
  • A first-time voucher recipient

NOTE: Voucher recipients are also required to attend the Chapter Leadership Forum held on the Sunday before the HFIC.


Once you complete and submit this form, an ASHE staff member will send the recipient the voucher within 5-10 business days. Note: Requests received prior to conference registration opening will not be fulfilled until after registration opens.


All voucher requests are due by June 30, 2025. Requests submitted after this date will not be fulfilled.


Vouchers for the 2024 Chapter Awards are only valid for the 2025 Health Care Facilities Innovation Conference.
