All Team Members shall be required to undergo the screening, vaccinations, and tests defined in the Occupational Health Screening and Maintenance policy.

• UVA Community Health requires influenza vaccinations for all team members, physicians, volunteers, students, contractors, vendors, and all others who provide services.
• External Flu Submissions: Team members who acquire their flu vaccinations from outside of EOH are required to submit proof of vaccination to Employee Occupational Health.

Exemptions: Team members must be formally approved for exemption by the exemption deadline date. Of note: To assess the request appropriately, team members must understand that they may be asked to submit a new application for religious/medical exemption in the future.

An approved exemption does not automatically mean your exemption will be approved permanently.
