Exception to Admission Policy Form

This petition process is meant to accommodate applicants with extenuating circumstances that may impact their admissibility to Ensign College. Before you fill out this request - please complete as much of your application as possible so the Admissions Committee can review your request holistically. If you would like to request an appeal to your admission decision, please visit www.ensign.edu/options-for-denied-students for more information.

Attach any supporting documentation to support and/or verify your extenuating circumstance. For example, transcripts, ACT/SAT scores, etc. If you are submitting an appeal to your denied admission, you must provide a Letter of Recommendation. Please note that this letter cannot be written by a family member.

You are encouraged to provide the Admissions Committee with new information that was not previously included on our application and information about your extenuating circumstances.
