Meal Pickup Form

There is no meal pick up on January 4. We resume meal pick up on January 5. This order form is for pick up of meals for anyone age 0-18. You will have the option to choose a school site for pick up or a bus route related to that school site. School site pick up of meals will be from 11:30 am to 1:00 pm daily. Be sure to check the link for the time of the bus stops. All orders will include a breakfast and lunch and will need to be picked up daily at the chosen location. All meals meet the USDA guidelines for Summer Food Service Program. This form ensures food will be available at the site when you come to pick up your meals. Please pick up from the site you have selected. You complete one form for the week unless you are requesting to pick up from different locations during the week. In this instance you will need to complete a separate form for each location you wish to pick up from. Durango School District 9R families are encouraged to complete a 2020-2021 Free and Reduced Meal Application even though meals are currently free to anyone 0-18 years of age.
