This form should be used by Faculty, APPs, Trainees, and Fellows to request funding for publications, abstracts, and professional development. The primary purpose of these funds is to support career development and faculty mentored projects. Requests by a single individual cannot exceed $5,000 in a fiscal year (July 1 - June 30). For Publications, if you have grant funding that would cover your requested expense, please use those funds. For all other requests, if you have other discretionary funding (startup, bridge, pilot, UW Health Professional Funds) or grant funding that would cover your requested expense those funds must be used first. These funds may be used for partial payment.
