Contact information submitted via this form will be used for correspondence regarding the NCJIS Modernization Program.
Submission of this form is not amending the contact information on file for your existing account at the Records, Communication and Compliance Division (RCCD), Department of Public Safety.
Please provide a contact for your accounting/billing information as well as a general contact for your company/agency. They can be the same person.
This form provides fields for up to three individuals of each contact type. If you wish to submit more than three of a contact type, please use additional forms. In addition, you may submit questions regarding the NCJIS Modernization Program on this form.

Criminal Justice Agency: If you wish to appoint a new Terminal Agency Coordinator (TAC)/Assistant Terminal Agency Coordinator (ATAC) or Agency Administrator, please submit a letter on your agency’s letterhead to mkdoiel@dps.state.nv.us indicating the new appointment.
