Fill out this form at the point that you have either completed all course requirements or are in the final course for the Graduate Certificate in Health Management. Please submit this form within 2 weeks of the start of your final course or your final semester if all the courses are completed. Eligibility: Be currently enrolled in a UC Berkeley graduate program; be in good academic standing with a GPA of 3.0 or better; and the three courses (9 units) must be taken for a letter grade.
