2025 SACNAS Chapter Annual Report

The Chapter Annual Report allows the SACNAS National Office to learn about the membership size, activities and financial state of each chapter, in order to better serve you.


This form is required of all provisional and official chapters in order to retain ACTIVE status and be eligible to submit a Chapter Award Application, a Community Gathering proposal , a COLOR self-nomination and eligible for more SACNAS programming/resources.


This form also includes the required 4 attachments to complete your report at the end of the form. Submissions accepted until June 8th!

CHAPTER ADVISORS

This section helps us keep updated records on your current advisors. Your chapter advisors are the faculty and staff that provide mentorship to your students and guidance on chapter activities. All your advisors should be have input in your submitted advisor report (Professional Chapters are not required to have advisors. Enter "N/A").

This section helps the SACNAS national office measure the size of each chapter. The end of this report will require a roster of your members. Everyone on your roster is highly encouraged to have a SACNAS membership. Sign up here.

Student Chapters: At least 10 members

  • Community Colleges and TCUs: At least 4 members


Professional Chapters: At least 5 members

Final Part - Attachments

For Student Chapters, this form must be submitted with all four attachments for your report to be considered complete.


For Professional Chapters, only the Membership Roster and Activities List are required.


  • ADVISOR(S) REPORT: This is a one page letter written by the chapter advisor(s). Chapters with multiple advisors should submit one letter that is collaboratively written by all advisors. The letter should detail how you provided mentoring to the chapter students, how you provided guidance on chapter activities, and how you promoted the chapter at your institution. (Not required for Professional Chapters)



  • INSTITUTIONAL RECOGNITION: Your student chapter should be registered and in compliance with all policies relating to student organizations at your institution. Submit a written letter or email from your institution that confirms that your chapter is registered as a student organization. Website screenshots are not accepted. Use or refer to this template as an example. (Not required for Professional Chapters)


  • ACTIVITES LIST: Include the name of the activities, date, and 2-4 sentences to describe the goal or purpose of the activity done from Summer 2024 to present. Include any planned for next year as well, if applicable. Use THIS TEMPLATE to submit your activities list.
